The Mental Health Counselor is responsible for engaging, providing assessment, treatment plan development, primary supportive psychotherapy, and referral as indicated for individuals dealing with a variety of mental health and substance use issues. She/he will evidence excellent organization and communication skills required to provide therapy within an integrated behavioral health/primary care setting As part of the Essential Function for this role, the Mental Health Counselor:

  • Assesses and provides short-term supportive counseling to individuals and families who are dealing with a variety of issues including but not limited to mental health, substance use, living with chronic illness including HIV, HCV, and issues related to sexual orientation and gender identity
  • Develops and implements treatment plans with an emphasis on evidence based practice within a harm reduction orientation
  • Provides resources and referrals as indicated to facilitate attainment of client’s treatment goals
  • Provides crisis intervention, supportive counseling (individual or group) and conducts mental health assessments and intakes for clients/patients of the Evergreen Association; Works within an integrated model coordinating treatment with providers involved in clients care both within and outside of the Evergreen Association
  • Utilizes electronic medical record for prompt documentation of treatment notes in concordance with organizational standards; Maintains confidentiality of treatment records

Qualified Candidate will have a Masters of Social Work or other related field with two (2) years of related experience. LMSW or LCSW preferred. Candidate must be able to communicate clearly and professionally in writing and verbally, demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills. Sensitivity to people living with HIV/AIDS, addiction, cultural and LGBT issues essential.
