Full Job Description

Position: HIV Case Manager II-Bilingual Creole

Classification: Full-time, salary, non-exempt

Reports to: Health Services Director

Purpose: To meet the immediate psychosocial and health needs of HIV positive clients at their level of readiness in order to restore or sustain client medical stability.

Essential Job Functions:

  • Maintain confidentiality by following all internal policies, securing doors, offices, and records of any client identifying information.
  • Complete eligibility determination for clients.
  • Establish a supportive relationship with clients and work collaboratively to identify needs, develop an action plan to meet those needs, and monitor client progress in service attainment.
  • Educate, identify and link clients to appropriate levels of health and support services necessary to enhance the quality of life, and maximize medical adherence.
  • Complete initial and ongoing assessment of the client’s needs and personal support system and document in client record.
  • Determine client’s ability to access services.
  • Determine what services client is eligible for based on program guidelines
  • Provide education on HIV, medications, safe sex, accessing services, etc., as needed based on client assessment.
  • Complete the comprehensive and individualized Action Plan.
  • Provide periodic re-evaluation and adaptation of the action plan making necessary revisions to match the changing needs of the client.
  • Assist clients with the application of all reimbursement services assuring that Ryan White is the payor of last resort.
  • Complete all documentation of services provided within 2 business days of contact with the client.
  • Ensure that all consent forms are current and in client records.
  • Track client adherence and compliance by recording medical appointments, laboratory results, and medication regimens.
  • Maintain client records to the standards laid out by the department and program.
  • Meet at least monthly for supervision.
  • Provide monthly reports on activities with clients, other providers, and knowledge learned during any training sessions.
  • Assist with set up and tear down for agency events.
  • Attend required and ongoing training.
  • Provide backup to other staff as needed.
  • Attend staff meetings.
  • Complete any other tasks assigned by Supervisors.

Qualifications:

  • Bachelor’s degree in human services or 3 years experience in human services.
  • Must be fluent in Creole/English
  • Experience providing HIV Case Management services preferred.
  • Knowledge of the needs of individuals living with HIV disease.
  • Demonstrated ability to work with a diverse client population.
  • Ability to communicate with a variety of health care and human services providers.
  • Strong organization, time management, communication, and interpersonal skills.
  • Ability to work under stress and with limited resources.
  • Ability to identify, prioritize, and manage complex health and social issues.
  • Maintain appropriate standards of care.
  • Accomplish problem resolution via team involvement and obtaining appropriate resources
  • Flexible schedule to meet client needs
  • Position requires occasional travel in the local area. Must have reliable transportation.
  • Ability and willingness to participate in ongoing professional development activities in order to keep current on new developments in the field and to enhance skills already maintained.
  • Attend and participate in community outreach and fundraising events as requested; this will occasionally require working additional night and/or weekend hours.
  • Ability to sit, stand, and walk.
  • Demonstrated ability to use Microsoft Office programs including Word, Excel and

Outlook. Working knowledge of web browsing and internet usage.

Physical and Mobility Requirements:

  • Position requires occasional travel in the local area. Must have reliable transportation.
  • Ability and willingness to participate in ongoing professional development activities in order to keep current on new developments in the field and to enhance skills already maintained.
  • Attend and participate in community outreach and fundraising events as requested; this will occasionally require working additional night and/or weekend hours.
  • Ability to be mobile throughout the day.
